The National Transportation Safety Board (NTSB) has concluded its investigation into the partial flooding of the bulk carrier Cuyahoga, which occurred on March 18, 2025, while the vessel was docked in Ashtabula, Ohio. The incident resulted in no reported injuries or pollution, and the vessel was successfully dewatered and refloated. The Cuyahoga had previously been declared a constructive total loss following significant damage from a fire in March 2024.
According to the NTSB’s final report, the probable cause of the flooding was identified as water entering the engine room through the main engine and generator water cooling systems’ duplex strainers. These strainers were reportedly uncovered while the vessel was in a layup status, and the ingress of water may have been due to a failure of the systems’ closed sea chest valve. The report does not specify the financial impact of the salvage operation, as the vessel had already been deemed a total loss due to prior fire damage.
